Collectly will send text or email notifications to you when you receive a new statement.

MedCura Health Sliding Fee Scale Program

For patients who meet the requirements, MedCura Health offers a Sliding Fee Scale Program. If you wish to apply for the Sliding Fee Scale Program you must provide verification of your current residence, a valid photo ID, and proof of income for the previous 30 days. Your family size and income will also determine if you are eligible for this program. Approved patients will need to recertify every 12 months to continue in the program.


What You Should Bring

  • If you have medical insurance, bring a copy of your insurance card.
  • If you have Medicaid or Medicare, bring a copy of your card with you.
  • If you do not have insurance coverage, proof of income for the past 30 days is required to qualify for our Sliding Fee Scale Program. Recertification is required every 12 months.
  • If this is your first visit to AvitaCare Atlanta, please arrive at least 30 minutes prior to your appointment time.
